Utah State Law requires that minors (those under 18) must be accompanied by their guardian in order to receive a piercing. This requires proper identification and proof of guardianship. Please present one of each item below day of service.
What do I need to bring to my child's appointment?
Parent ID : Government issued identification.
Child ID: Such as government issued ID, school ID, school login with photo and name visible, or copy of child's immunization records.
Child's Birth Certificate or Proof of Guardianship: Must show guardian's name and child's name on record. Custody agreements and guardianship paperwork must be legally notarized.

How old do do you have to be?
Ear lobe piercings - approximately 7 years old
Oral and Facial piercings - approximately 14 years old
Navel Piercings - approximately 15 years old
Other piercings - approximately 13 years old
Surface piercings, Tongue and Intimate Piercings 18+ No Exceptions!
